Indigenous Community  Supports  Navigator (ICSN)

POSITION SUMMARY:

The Indigenous Community Supports Navigator (ICSN) will liaise directly with the Canadian Mental Health Association (CMHA) Adaptive Case Management (ACM) team to support Indigenous families through ISP review and ongoing support, rapport building, and participation in Integrated Coordinated Access Partnership when needed. The Navigator will be responsible for assisting the supervisor in the development and provision of Indigenous competency training to other service providers as requested.

DUTIES AND RESPONSIBILITIES:

Through a person-centered team approach the Indigenous Community Supports Navigator will:

  1. Provide Case Management Support & consultation to Indigenous families through the ACM team by providing lived experience of Indigenous supports and navigations specific to the Indigenous population.

  2. Integrated Coordinated Access Support- provide system navigation and referral specific to Indigenous families. (This requires the completion of Tier 3 systems level navigation training).

  3. Competency Training- Develop Indigenous competency training to other service providers and other Indigenous serving organizations in the community as requested.

  4. Best Practice, community needs and knowledge translation- complete the City of Lethbridge Best Practices and Research training (annually) while keeping updated on national best practices, community trends and develop and disseminate reports on a quarterly basis that current best-practices across Canada, community trends, mitigation strategies, and avenues for further research and/or programming and supports in the community. Dissemination of these reports to the City of Lethbridge- Community & Social Development office, CMHA, and any other Indigenous-serving organizations at their request.

 

REPORTING LINE: 

The Indigenous Community Support Navigator is directly accountable to the Family Preservation Supervisor

RATE OF PAY: 

$24-26.85/Hour + Benefits

QUALIFICATIONS:

  • B.S.W. or other Human Service degree/diploma, combination of education and experience

  • Experience working with Indigenous families

  • Experience working with service agencies within the City of Lethbridge

  • Awareness and understanding of Indigenous culture and issues

  • CPR & First Aid certification

  • CYIM & Criminal Record Check, 6 months current                                                        

  • During COVID-19, will be required to adjust work conditions
